IMPORTANT NOTES

¹ This premium rate is applicable for Class 1 occupation, under Plan 1 coverage.

² For weekly cash benefit, you can choose to insure up to:
– 0.7% of the sum insured for accidental death, or;
– 20% of your monthly earnings, or;
– $1,000 a week,
whichever is lower.

³ Your sum insured must be equal to the accidental death benefit.

⁴ You can choose to insure up to 0.1% of the sum insured for accidental death or $300 a day, whichever is lower.

⁵ You can choose to insure up to the sum insured under accidental death.
